These terms explain the basic rules for using Trusted Partner Forms and the related request pages.
Use of the service
Trusted Partner Forms helps organizations collect requests from clients and make introductions to service providers. You agree to use the site only for lawful, accurate, and respectful communication.
Eligibility
You must be at least 18 years old to create an account on this platform. This site is not directed to children under 13.
Acceptable use
Organizations that create forms on the platform agree not to:
- Use their form to request or collect sensitive personal information, including Social Security numbers, government ID numbers, financial account or payment card numbers, passwords, or health or medical information.
- Use the platform for any unlawful purpose, or to offer or refer services that are illegal where offered.
- Use submitted contact information for purposes beyond the introduction the submitter requested, or add submitters to marketing lists without their separate consent.
- Send spam, misrepresent their identity or affiliation, or impersonate another business.
- Attempt to access data belonging to other organizations on the platform, or interfere with the operation or security of the service.
Introductions are not endorsements
A trusted partner introduction is not a guarantee of availability, quality, pricing, licensing, insurance, outcome, or fit. You are responsible for evaluating any provider before hiring or sharing sensitive details.
Account access
Dashboard access is provided through secure email sign-in links. Keep access to your email account secure and notify Ellis Taylor Marketing if you believe your dashboard access has been misused.
Submitted information
You are responsible for the accuracy of information you submit. Do not submit payment details, Social Security numbers, full legal case files, medical records, financial account numbers, or other unnecessary sensitive data.
Availability
The site may be updated, paused, or changed at any time. We work to keep it useful and available, but we do not promise uninterrupted access.
Suspension and termination
We may suspend or terminate an account, or remove a form, at any time if we believe these terms have been violated, if the account poses a risk to the platform or to the people submitting forms, or if required by law. Account holders may stop using the service and request deletion of their account at any time by contacting privacy@ellistaylormarketing.com.
Disclaimer of warranties
This service is provided "as is" and "as available," without warranties of any kind, express or implied, including warranties of merchantability, fitness for a particular purpose, and non-interruption. We do not guarantee that the service will be error-free, secure, or continuously available, and we do not guarantee any outcome from any introduction requested through a form. We are not a party to, and are not responsible for, any relationship, agreement, or work performed between a form submitter and any organization or service partner.
Limitation of liability
To the fullest extent permitted by law, Ellis Taylor Marketing will not be liable for any indirect, incidental, special, consequential, or punitive damages, or for lost profits, lost data, or business interruption, arising out of or related to the use of this service. To the fullest extent permitted by law, our total liability for any claim arising out of or relating to this service will not exceed one hundred U.S. dollars ($100).
Indemnification
Organizations that create and publish forms on this platform agree to indemnify and hold harmless Ellis Taylor Marketing from claims, damages, and expenses (including reasonable attorneys' fees) arising from their use of the platform, the content of their forms, their handling of information submitted through their forms, or their violation of these terms.
Changes to these terms
We may update these terms from time to time. The "Last updated" date at the bottom of this page shows when they were last revised. Continued use of the service after changes take effect constitutes acceptance of the updated terms. For material changes affecting account holders, we will make reasonable efforts to provide notice, such as by email or a dashboard notice.
Governing law
These terms are governed by the laws of the State of Maryland, without regard to its conflict-of-law rules. Any dispute arising out of these terms or the service will be brought in the state or federal courts located in Maryland.
